Position Summary
The Case Manager/QBHS will provide trauma-informed, culturally responsive support to
children, families, and community members engaged with Rainbow of Resilience. This role
bridges behavioral health, educational, and legal advocacy systems, ensuring clients receive
coordinated, accessible, and effective services. The Case Manager/QBHS will serve as a
navigator, advocate, and connector—guiding clients through service systems, strengthening
protective behaviors, and promoting emotional literacy and self-advocacy.
Key Responsibilities
Client Support & Advocacy
- Conduct intake assessments and develop individualized service plans.
- Provide ongoing case management, including monitoring progress and adjusting plans.
- Advocate for clients across healthcare, education, legal, and community systems.
Resource Coordination
- Connect clients with community-based supports including housing, healthcare, education,
and legal aid. - Maintain updated resource maps, referral pipelines, and partnerships.
- Collaborate with agencies, schools, hospitals, and community partners to ensure
comprehensive, wraparound support.
Trauma-Informed Practice
- Apply culturally responsive, trauma-informed approaches in all interactions.
- Support clients in building emotional regulation, communication, and self-advocacy
skills. - Identify triggers and help clients implement safety planning and coping strategies.
Program Integration
- Incorporate Rainbow of Resilience’s VR-based SEL curriculum and CBT tools into case
management practice. - Gather client feedback to support program improvement and curriculum development.
- Assist in piloting digital products, documenting outcomes, and supporting innovation.
Documentation & Compliance
- Maintain accurate case notes, progress reports, and compliance documentation.
- Track outcomes using dashboards, reporting tools, and data systems.
- Ensure confidentiality and adherence to Medicaid, agency, and ethical standards.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Counseling, or related field (Master’s
preferred). - Minimum 2 years of experience in case management, behavioral health, social services,
or related support roles. - Knowledge of trauma-informed care, protective behaviors, and culturally responsive
practices. - Strong communication, advocacy, and relationship-building skills across diverse
populations. - Familiarity with digital tools, VR/technology-driven interventions, or data dashboards a
plus. - Ability to effectively navigate healthcare, education, and legal systems.

Compensation
$25–$45 per hour, based on experience, credentials, and scope of responsibilities. Contracted team members may also be eligible for performance-based incentives and project-based opportunities as the program expands.
